Instructions:

  • 1. Complete this form and fax us a copy of the MSDS sheet prior to shipping materials.
  • 2. Send (prepaid) 5 cu. ft. of material in returnable container(s).
  • 3. Attach a MSDS sheet to the outside of the shipping container.

    • Attending the test:

    We are located in Port Hope, Ontario, 60 to 90 minutes drive from Toronto International Airport. If you wish to attend the testing of your material, we suggest the following accommodation:
